Yes I have seen this happen lots of times, but only when way over the side - say working over the passenger side of the truck and the outrigger on the driver side will be off of the pad every once in a while
I would suggest assigning somebody to grease duty and make sure the drivers are checking fluids and doing circle checks- we have our mechanics spot check the trucks routinely to see if the guys are missing things. I find the threat of hedging gets people doing circle checks!
Do you have any formal training? Some times a bit of education regarding decay/hollows and strength loss can go a long way with putting the mind at ease. Might be worth looking for a workshop or two to attend.
Raise the pad up about half an inch higher than it is and it makes a huge difference, I went from caddy pads to the steins and almost threw them out of the tree on my first climb. Did a bit of research and read about raising the pad and I have been happy since- about 2 years. Good luck
