Identify Please

I'm located in Oklahoma USA and this tree is around 25 ft tall and approximately 5 years old

That was my thought too. Cultivar is more difficult, but I'd bet against Allee because the bark would be flaking off more at that diameter. Maybe Frontier??? My neighbor has one that looks about identical to that size/shape and bark pattern development.

I'd also bet the landscape blocks are filled with soil and mulch covering the root collar...

Yes thats bad! should dig down a few inches to determine that. If so the owner should be informed he may rot the collar.
