Sony low what thread to put this under

I'm getting hooked up with a website for almost free ( have to pay domain fees and something else) but all the designing is free. I've help a friend of mine out for years, cutting her firewood up for her. She designs websites on the side. She offered to ale one for me. Anyways. Content ideas have been coming in, but I have one thing I want to have on there, but have no idea how to do it, maybe she does, but I figured someone here already has it on theirs.
I want my "request a free quote" links to lead potential clients to a former to fill out the details of their potential job, hopefully with some detail, and maybe ability to load pictures.
Anyone have something like this already, is it a stupid idea? I thought it would be helpful, but I can also see where I don't NEED it, I can just go out to the job site . Thanks in advance

Depending on her platform - Wordpress, Drupal, Joomla, etc.; there will be a plugin for contact forms that can be customized. Contact Form 7 is what I use on my website.

I've found though, that the simpler the form, the more leads you'll get. If it too long and detailed, they'll skip it.
