There is no doubt that gear should be inspected frequently, however, in my opinion, it seems that the bigger issue here is that RE was using a set screw as opposed to drilling and pinning the axle assembly. This, in itself would have completely alleviated any potential issues. In my mind, this is a huge red flag for this product. I understand that the set screw was put in place to make the unit rebuildable, but in this case, a set screw is the wrong way to go. It just goes to show you that even a big player like RE has flaws. This is a major issue. A set screw in a product like this is just asking for problems.
