Planting and mulching are def in the planting standard, transporting may be in that one too. BMP is with it too.

Does anyone think there is a bigger problem or equivalent to mulch volcanoes?

I’m saying that mulch volcanoes are the biggest problem I see as far as mulching goes. Does anyone disagree or think something else is more of a problem with mulch?

Buried root flares call also arise from deep planting. I also see more nursery stock with buried root flares than those that aren't. The root ball ends up being well undersized by the Nursery ansi when you're done digging it out.

Growers can do way better, as can installers and maintenance folks.

Agreed but I’d label that a planting issue more than a mulch specific issue. But I agree all the same
