Spectacular chipper disk failure

Been trying to find the full story about this but suffice to say from what I've heard and read here the chipper had been substantiaallly modified and abused.
The verbal story I heard was that the disk exploded out of the chipper and at least part of it went through a backhoe cabin and then on to a stationery vehicle where it went through the passenger door and killed the fellow in the driver's seat who had apparently had something to do with the chipper's poor state of modification or abuse.

Tom forwarded me a link to the report...and there were citations that the blade slots were either modified to accept a larger knife or that welding was performed to reinforce the outside end of the slots. In other words, the were messed with.

First thing I did was go out and look at my wheel and check the load bearings again.
