@Steve Baker; I think it stays together simply because of a combination of a tight fit and pressure from being pulled on.
The Rope Wrench is not life support, so even if his pin fell right out, it would not cause a fall. Your hitch would just bind up tight because of the loss of a tool to share friction with the hitch.
If I'm remembering correctly, it has not yet caused an issue for @ClimbHy.
Hopefully I'm not too far off with this explanation.
