Question about brakes on big trucks

This seriously worn-out disc brake pad fell of the garbage truck in front of our house. It was still very hot and no other trucks had gone by. I called the city solid waste department to report it so they could get in touch with the driver. I found out later he had driven several miles to the landfill and then to the maintenance shop.


How was it possible for the disc to get out of the caliper? How much additional damage would be likely after continuing to drive the truck several miles?

The rotor is wrecked for sure. I don't know enough to say more for certain, but I'd bet the rest of the brake system should get a good looking over
