The older buckets aren't that fool proof in my mind, they don't have all the new safty features. I've done the same thing thou, lifted up the boom to get into a box and once it was up I assumed the outriggers were out. It can happen to anyone as much as we may not like to admit it. The newer buckets are much smarter, unless all the outriggers are down the bucket won't move from the controls.

First thing I noticed (like any one here would) is the out riggers not being out. The thing that bothers me is the article says they didn't know why it happened. Well, there are other arbos out there, why didn't they ask them?

Another sad story.
