Very sad accident today...

After pulling lots of bridges and testing the roll and lock I have switched over to the non-sexy good old 16 braid arbormaster. It's not expensive, it's not complicated and it is always what you see is what you get, no hidden secrets inside.
Do you remove the core of the 16 strand so it will fit into the holes on the lower d's of the TM?
 
It didn't look like any limb removal had been done on the tree. My guess is the transition from the pill to the tree didn't work out.

I want to know, like everyone else. However, getting hit on the head by falling debris is still the top killer in the industry. If we pursue this important iota about this incident, we should also, and first, make sure that our game is solid concerning the documented top causes of death. Otherwise we're kind of rubber necking...

And something I try to tell new workers for they get out there in the field the number one way of getting killed in this business is to walk on and eat that tree and get hit by falling branch

Wakes them up to the danger
 
The point of learning the details of this accident is to give provide us with a learning tool. Instead of speculating on the exact cause we will know and thus can learn and teach. It will no longer be hearsay or our jnterpretation that can then be refuted and undermined.
 
Like what? "Certified techniques" or plain stupidity?
I won’t call it plain stupidity, that infers a lack of knowledge or inability to grasp a concept.

Certified techniques? Hmmm. I’m not 100% sure on this descriptor, techniques that are set forth in a standard, ie., ANSI Z133 ? Those are the certified techniques that any investigation will look at and reference.

From no pre-climb inspection, no second tie in or work positioning, one handed use where two handed is easily done, no call and respond, disregard for ground crew movement, etc...

All justified by the done it before or safety slows me down line.
 
I'm sorry about your friend. Was he just an employee or a friend? How long has your friend been accepted from his death? Just my friend told me that a long time ago his employee lost his wife in an accident. I was shocked that he fast accepted from her death. A month later, he met a new woman. His co-worker met her thanks to Dating Successful Women. For me personally, this is a shock. How can he so quickly forget the wife with whom he lived for about 5 years? It is clear that he had conflicts with her, but they were together for so long.
