The biner was alright when I checked it on the ground. Fortunately, the biner just connected the pulley to the ascender and was not in a life-support position.
We did one pitch and the biner seemed to be working at that point. I think, the trouble started when Lance tried to remove the biner to descend. Hmmm, you know what, I've got the entire climb on video from my helmet cam; maybe I can see where the trouble started.

I just reviewed my helmet cam footage. The biner was closed as it should be all the way up. As he prepared to decend, Lance removed the pulley from his ascender, and handed me the biner. I immediately saw the gate wasn't closed. Soooo, now I'm thinking one of two things. One, something got in it and jammed it, or two, Lance forced the gate open without fully twisting the gate, or something coincidentally failed in the biner as he started to remove it. Yeah, I know, that's three things.
I'm kinda thinking the second possibility is the most likely one. Lance hadn't had a lot of practice opening the gate and he may have forced it open.
