Lull drops Crane

I'm not a crane expert by any means but how could they not see that coming with the angel they had it held. Its simple physics that it was going to tip some time or another. Is it me or does common sense provide you with a nicer life? I've never done a crane job so I have no idea. But isn't feeling it out a big deal? I've seen a guy or two in my company drop a top onto a crane and I've ran out of the way because it bounced a bit. That didn't even bounce and I wanted to run.
