JOB SITE ASSESSMENT FORM / BIDDING FORM ???

I just started my own business and am putting together a job site assessment form that will be used while bidding. I am hoping to see some examples or get some suggestions from you fine folks!

I am basically wanting to put together a checklist that I can walk through while inspecting a tree/job
 
I made one for someone bidding jobs for me a while back, but don't see it in my files.

I had a checklist and blanks for inputs.

Checklists of equipment and vehicles needed.
Things like access concerns (width of gate, etc), length of drag.

Septic
Power (service drop, and any additional, such as a direct bury line to a subpage, such as good to my pole barn, 6-10" deep---shallow!)
Irrigation
Internet
Gas line
Water line

Ask if they plan to change things between bid and job.

Ask what is worth saving/ can be damaged.
When they say that they'd like to keep the bush beneath the tree, I then ask if it's with the $300 extra to work around it... things become expendable fast.

Note conditions, like fence with broken posts, house has scratches near gate. Gate was gouged, previously.

Protect anniversary present Japanese maple.



Note nearest emergency facility.


You need to get employees to sign and date something that said they reviewed the Job Hazard Assessment, understand the dangers and mitigation plans, understand the job, understand that they can stop the job at any time for safety, and have had a chance to ask questions, and have had them answered.
File it!
