Health insurance from company

How many of you out there give health insurance for your guys? I am working on setting up a program up for my guys, but have questions.

What types of insurance do most people carry?

I have a team of three right now but am looking to add to that in the future.

Do most companies cover 100%? 50% what are you guys doing?

Does anyone layoff guys in the winter and bring them back every year? Do you still cover them during the layoff?

What about dental and vision?

Any info anyone has would be of great help, thanks

I wish my boss helped with ANY of my insurance. We are a company of 3(including the boss/owner/climber). After 8 years, 2 sick days and no unexcuses absences, it would be nice.
