gettin a bit personal, aren't we gentlemen? this thread is starting to sound like another forum i used to visit a few yrs ago.

let's keep the air clean over here - more information gets through that way :)

The climbers tie in is a big mistake in more ways than one, that is quite obvious.

It is a little tough to tell from the pic because we do not have the perspective of the landing zone. so i'm going to hazzard a guess and say he also set up too take to big a wack. There looks to be a house tap (electric line) running under the lowering point. so i say that limb should have been taken i 2 pieces at the Y to reduce the risk of hanging it up on the house tap.

Also is there a bucket in the lower left corner?
