digital pre-job brief

Trying to formalize and record our pre-job briefs. As of now, I came up with a form that I then laminated so it can be reused. However that makes recording it difficult, short of taking a picture of it. I'd like to avoid a paper form due to clutter/lost paperwork. If possible I'd like to find an app that can act as the form and then somehow be saved digitally, preferably that can be used with a phone? Kind of a vague description I know but does anyone have any experience with something of this nature?
 
Google docs. Saves the input to a spreadsheet. Its what I use for all my crane inspections/DVIR/Job site assessments. Super easy and you can save a link to the notes in your iPhone and select it to open a new form. Super easy
 
I was thinking Google Forms. I'm not sure how a Google Doc can save it's input to a Google Sheet -- but the Form can. I use one to record milage for the business, fuel mileage on all the vehicles, charity driving, etc.

Not sure about the signing part -- that might require an actual app.

You can use Adobe Acrobat to sign PDFs. My wife has used this before. Looks like for mobile users, you'll have to use an email link, but you'll get a signed pdf from the recipient.
 
Thanks for the replies guys. I work for a municipality and through GIS was able to create a survey that's accessible through the related app. Should streamline the process.
