    Condolences to his family. Unfortunately there is scant information that we could learn something from this. What gets me is calling it a freak death. It’s a preventable incident and needs to be viewed as such.

    Your policies need to be aligned with industry standards. When something goes wrong then that is how the courts look at it.

    I’ve had every version since 2007. It’s crucial to writing a safety manual as it is the reference that a court would use along with OSHA for industry standards.

    Good points pertaining to use of systems. Licensing to require minimum industry standards and compliance when enforced is one way to address some of the issues. Mandatory training on effective use of current systems that demonstrate consistent safety. Then we could better assess the...
