Roger that...not with a zip line, but I have seen it happen in rappelling...had a girl not put her long brown hair up out of the way properly. Her hair fed right into the biner wrap (old days, no figure eights, etc. then). She was trapped until her hair was freed from the biner...not a happy camper!
We also always used a double rope rappel (and still do). We had a fellow (about 40 feet up) get his shirt caught in the biner wrap. While the Rangers were deciding how to extricate the fellow, Mr. Ghetto whipped out a BIG knife and cut his shirt free...he also cut his rappel rope. The second rope saved him...at least from falling. The Rangers were waiting for him when he dared to come on down.
