Why did this fail..?

I'm glad we can use the internet to learn from each other and not put people down.

I would suspect the internal decay down low inside the trunk combined with a high wind event. To avoid falling with the tree use a rubber mallet to "sound" the trunk and identify high risk internal defects - and don't climb during high winds.

Climb safe and keep asking questions.
 
Until you've blocked down a lot of trunks and looked at the cross sectional dissection you might not have a great idea what a thump means. With attention though you can have a bit of an idea.

Another way to learn about thumping is to go to the local wood dump and thump logs. Ask a local tree service if you could meet them on a job to thump chunks. Bring your PPE along

There is a noticeable tone difference though. If you're paying attention you'll hear it.

Not a bad idea to do thumps all around a trunk on the way up. That's sOP when climbing dead or dead-ish trees
 
When you've climbed a lot of sketchy trees, you will get a feel for what doesnt feel right. The "s" wobble in the trunk below you, the more than anticipated sway...
I've been up trees that looked visually ok, but were basket cases, and vice versa.
Sometimes this involves a willing suspension of self preservation.
