How many folks have routine training sessions for the crews? Something along the lines of the TCIA Tailgate Safety Program. With Tailgate, there is a test. What pass/fail rate to you set for the crew? It seems that it should be 2/3 at a minimum.
Then, how do you deal with the ones who fail the tests? Is there remedial work? I know that some people don't read either English or Spanish so reading the test outloud is a possible solution.
If records are kept for the pass/fail and it seemed that the same people were failing more than others, how would you deal with that?
In ANSI Z-133 4.1.2 it states, "Employers shall instruct their employees in the proper use, inspection and maintenance of tools and equipment, including ropes and lines, and shall require that appropriate practices be followed." There are other regs that relate to required training but I haven't ever seen any pass/fail parameters or remedial recommendations.
