I think that a specific tree defect section would be important, especially if the Job Site Inspection form began with a salesman's visit. Also, note if there might be any unusual obstructions to observing defects (significant ivy or other vines, or on a steep hillside/ cliff where it may be difficult or impossible to get a 360 view of the trees. Fruiting bodies may be visible at the time of the initial inspection, or reported by the homeowner. Active cracks or bad inclusions/ unions that might factor into rigging forces which can be discusses at the pre-climb work planning between the appropriate people (foreman, climber, bucket op, ground crew).

Cavities that might have bee hives/ animals
Roadway/ pedestrian traffic/ loose animals like the free range neighborhood dog/ exiting people from buildings like someone leaving the house/ apartment building by a side door and coming into the work area
