COVID Vax and HIPAA

The vaccine itself is free, but the pharmacy or other provider may charge a nominal fee for giving you the injection. Most insurance plans will cover it. I got an EOB (explanation of benefits) from Medicare showing they paid a claim of $28.39 for anesthesia when I got each shot at the local County Health Department. I didn't pay anything out-of-pocket.

The provider can get reimbursed for uninsured patients through the Health Resources and Services Administration’s Provider Relief Fund according to the CDC. It's easy enough to ask about the out-of-pocket cost up front, though.
