The mushrooms... I think I'd lose my license if I claimed that I could firmly ID on the basis of that photo. Sorry, not trying to be snarky. Might actually be the same as the first tree in the thread. None of the key characteristics can be seen!

yes it is the same tree, and fungi as the first photo. The second photo and cores done about a month later after the first frosts of the year are just a follow up.
 
So here is the last follow up. We were just hit with a big wind storm. Home owners noticed a new lean and soil cracking.
They had a crew remove the tree ASAP.
