alright I'll post two.
This one happened only 3 weeks ago:
We have a GMC dually 3500 pick-up with a dump insert.
Truck is parked in the driveway of a customer, facing outward toward a city street.
Truck is a stick shift.
I was loading logs into the truck from directly the rear with my skid loader and grapple. So I'm stuffing them straight in (like the attached pic).
No one is in the truck, it's not running and it's in 1st gear.
Well, one of my guys doesn't know to turn the key back I guess to assessories to listen to the radio. He had been listening to the radio earlier with the truck off, but he had turned the key to the "run" position. (a first).
Loading a log, I was a little low coming in and bumped a log on the bottom row. This shoved the truck forward just enough to turn the engine over. The truck started itself and begun a rough bucking run towards the busy street.
The man that had left the key on was nearby and opened the door on the run and stopped the run away truck before it got to the street. It wasn't going fast cause it has low gears and wasn't running on all cylinders yet. But still that was scary.
I just laughed cause what are the chances of that happening? I could have gotten mad about the key being in the run position, but I didn't.

Next one:
about a year ago.
taking down a large sycamore with many house obstacles and many many utility lines.
came down out of the tree for lunch. my wife had stopped by. the last 20 feet of the decent I pushed out and was just going to drop to the ground fast (to show off I guess). well my tail of my climbing line wasn't routed where I was going and my prussik locked up.
I had needed to drop to the ground or else I would likely swing out to the house if I stoped because my tie in point was out towards the house. Since I didn't get to the ground it swung me toward the house. I put my foot out to cushion my bump against the house, well my heel broke a cheap $12.00 fake shutter. I returned that day with a new plastic shutter to replace it.
They are attached with these plastic plugs that have barbs. I was pulling the plugs out with a pair of pliers. Picture this, I have a pair of pliers on a plug, both hands on the pliers in a hammer fist position, pulling really hard. Plug brakes off and fists and end of plier handle hits me in the top of my forehead, really hard where my hands bounce away. Instant fast flowing blood runs all down my face. Deep hole in my forehead. Go to the truck, clean it off, hold it shut and apply the best thing in my first aid kit, superglue.
boy, that feels so stupid to punch yourself in the head. there was no stopping it though
