Man Thursday sucked big time.
Stepped some stinky dog shit that stuck with me the hole day and then this...

Lost bolt, nut, whatever was in the bottom hole.
The black glue there doesn't stick to the ascender at all.
No locktite?
Guys check and tighten your bolts and put some locktite in there.
 
I got it last year and used it on two jobs and on about 6 jobs this year.
One thing I noticed when I put it back together is that if the top screw is tightened down all the way tight it would interfere with the cam on the ascender.
I put a washer and lock washer for spacing.
Makes me wonder if that screw was only screwed in so far when assembled.
A shorter screw tightened down tight with some locktite should have been used.
This could be an issue if everyone that was made used the same length top screw as mine.

Yes I do have the replacement screws available on my website and they are of the right length. There's not a lot of threads in that sscender and they are assembled with Loctite but like anything that moves, if they're not checked they can come loose. I don't know if you've ordered them yet if not private message your address and I'll just send you some.

One other thing, the black plastic is not intended to actually glue and hold the ascender but just be a spacing material between the tubes.

AM I correct that the top screw is too long to be tightened all the way?
Seams to me that a 1/4" screw would be a safer fit as the head of the screw could snug down against the webbing and add friction to prevent it from coming loose in addition to the locktite.
The only thing holding the device together was the small sewn loop at the bottom of the ascender.
I consider that a testament to your sewing machine.
