We all just need to educate our customers one by one. Tell them that this practice is unacceptable for thier trees. If they want to listen to us then great one more person to spread the word about mulch volcanos. If they don't, its there landscape, and thier problem. I had a customer who had just had thier trees mulched, called me to ask why the trees were doing so badly ("Ive mulched them every year"). I got there and suggested the removal of the last 10 or so years of mulch (2-4" each year), cut the girdling roots and re-apply it correctly. This cost them 500 for 8 trees. I also suggested that they find another landscaper (and suggested a couple I know).
