Near misses are the warning flags and free opportunities to review practices and more importantly,underlying conditions that are leading to potentially unwarranted risks.

Creating a Q&A form that helps to analyze an incident is hugely beneficial.
 
I've messaged the ministry of labour and the ihsa so hopefully I get some more ideas and get the ball rolling on a site soon enough

A q&a is great I'm just trying to compile enough good questions to be sure we ask the right ones. I have a very basic layout in this thread but it is just scratching the surface.
 
So far I got a copy of the curriculum from the Ontario colleges. As well as a mol inspector got my email and said it could be handled better by someone else he works with so he's passing it along.
 
It would be helpful to outline some of the questions to be asked in an interview or even to organize them by field in a form such that their is a searchable database.
How would a near miss form differ? Or could that be part of this form?

I was actually referring to your questionnaire for the accident investigation. However, in a job interview scenario I'd ask the following:
  • Describe the tailgate meeting you've run or participated in?
  • Describe a situation when you didn't wear any part of your PPE?
  • Is there a time that you didn't think it was necessary to wear your helmet?
  • Can you tell me about a time where you saw someone doing something that goes against safe work practices?
  • What did you do?
  • Have you ever had to refuse to do a job that you felt was unsafe or recommend a different approach?
