Equipment Inspection

Just wondering what stories you all might have about equipment checks. Things you have seen that would have eventually turned dangerous. Concerns you have etc.
I noticed today that one of the allen screws in a shackle holding the bridge to my saddle was working its way out. I do a visual inspection before each use but I don't take a wrench and tighten these things each time I use it. Had I not noticed it could have been bad when that bridge came loose.
I took both out and re-secured them but with thread lock.

I have threadlock on the shackles of my Tree Austria and I check them with an allen key once a week and visually inspect them every day. I put a paint mark streak that hits the bolt and the housing as well so visual inspection is easy. If the paint mark is uneven, then the bolt has loosened. Just a good way to check quick with a glance daily.

Great idea on the paint mark, I'll do that! I wonder why the manufacture does apply thread lock.
