Sad to see. I know that we all have "that" saddle, but did you see the image for "FIG 2" It wasn't the cause of death but from the looks of that saddle it very well could have been....
The saddle condition is irrelevant in this case. How ever if I were the owner of a company I would question the judgment of someone willing to climb with that geriatric gear.
This is the third time this scenario has played out with the same tragic ending. Hiring "experienced workers" doesn't negate the need to train. It in fact can double the effort of training because of the need to unlearn old habits before acquiring the new ones.
This report is immensely useful for us. It would be good to see these on a regular basis and particularly those that are for the accidents that have been posted in Awakenings.
