CMC fatality

I received the pictures of the failure of this incident. The failure of the lift is located in the same area as other failures also where cmc sent a letter to inspect and they would fix it I believe….This needs to stop. I would avoid this specific lift. Not worth the risk. Im fortunate enough to survive from this same failure. Condolences to his family. ☹️

Does the upgrade fix the issue in question?
 
CMC and all access point the blame on the operators. Obviously their booms are not engineered properly (under built).

What pisses me off is that I met with the owner of all access years ago (Lenny). He claimed that the CMCs are the most “rugged” compared to the other manufacturers. In an add in TCI mag he even mentioned other manufacturer’s lifts as being “wimpy.”

Well… I haven’t ever heard of any booms breaking on other brands of spider lifts or man lifts and now a man is dead. Others have been seriously injured.

Reading the links above, does inspect just mean visually inspect or have NDT (non destructive testing) done?
If my goolies were depending on these things, I'd want metal fatigue testing done in a known failure area and KNOW my lift was OK. But honestly, I still prefer to be tied into a tree knowing what is going on in industry with counterfeit Chinesium parts and a whole range of issues that are driving manufacturers and maintenance folks crazy these days, to be fair to them. Cost cost cost . . . . sez the spreadsheet guys in their cozy offices disconnected from all reality.

What is the background on these machines?

Overloaded?
Rolled?
Rigging/ Craning with the machine?

Full- strength cut and chucking (dynamically loading the basket/ boom)?
I think the cut and chuck thing could be a consideration at play while using one of these lifts…..weight of operator, then add a swinging limb, or piece of chunk wood then throwing it…. I personally have ZERO experience on one, but they just don’t look sure footed or stout in my opinion. Hell I get butterflies when I’ve got our bucket truck in certain positions.
 
We have had our 83hd shut down since late February/early March, after it was inspected following the incident in Wisconsin. Cracks were found in the same location these failures have occurred. All Access is trying to put lip stick on the pig. Not good at all, and shame on them.
