2 days ago a BC climber fell while bucking down a spar. The investigation is on going.
So far it seem that he had a crimped steel core lanyard (not a spliced one) and the crimp was rusted and not in good condition. That made it the weak link in his chain and where his system failed.
I don't know if a rope was cut or if he wasn't using a second point of attachment and don't want to speculate. What I do wan't to say is inspect inspect your gear and your crews.
The MOL here say they like to see records of monthly ppe checks, dig out and brush up your safety procedures and learn from this.
