Site safety form

Hey guys,
I am looking for site safety forms if anyone has them. My old company used to use them and I would like to implement the practice in my business as well. I looked on the inter webs and found some that were either too specific or too general. Does anyone have one they use and like? If it is proprietary could I ask for a copy? I would gladly buy a beer or drink at Expo for a copy of that form

Hi @speelyei Where did you/ do you get that form? I don’t mind trying to recreate it but am not that technologically proficient and it would take a bit. Thanks!

That was an Asplundh form. Ive got some paper copies. I have a few others that are similar. Send me a message with an email and I’d be happy to send a pdf. or a word document you can print.
