Hey everyone! I have been looking around trying to find a job site brief/hazard assessment form for a while, but haven't really come up with anything that fits yet. I was hoping some of you would be willing to share what you are using or point me in the right direction. Thanks for the help.

I am looking for a form to be completed in conjunction with the ANSI mandated job briefing that is supposed to take place before each job starts. I have seen examples of these forms before in conferences and the like; they usually include a hazard assessment of the job site.

X2 on the TCIA form. For tree care I use this form, but it's more tree-oriented than job-oriented. TCIA is the best source for managing jobsite activities.
